A 2022 study of 131 postmenopausal women found something specific. The women who had fatty liver had significantly lower blood glutathione, the antioxidant the liver depends on more than almost any other organ.   • A quiet note on an annual physical you almost didn't ask about
  • A scan ordered for a completely unrelated reason

Your weight has not changed dramatically. You are not a heavy drinker. And yet there it is, hepatic steatosis, a clinical name for fat that has built up inside liver cells.

What almost nobody explains is why now, after decades of the same liver working fine. The honest answer starts with a hormone most people associate with hot flashes and bone density, not the liver at all.

Every cell in your body makes glutathione, a small molecule built from three amino acids that functions as your main internal antioxidant.

Glutathione does four jobs that matter here.

  • Backs up detox processing. It is a direct participant in how the liver breaks down and clears what passes through it.

As glutathione falls and oxidative stress climbs, the disease tends to track right alongside it.

What the animal research found

In one study, researchers found that removing the ovaries in mice lowered a key glutathione-producing enzyme in the liver and raised the oxidative damage measured there, while restoring ovarian signaling raised that same enzyme back up. A separate study in aged rats found that estrogen supplementation improved the balance between oxidized and reduced glutathione in the liver and reduced oxidative damage compared to untreated animals.

Both are animal studies, and we want to be direct about that rather than blur the line. But they describe a specific, testable claim. Ovarian hormone signaling appears to help maintain the liver's glutathione defenses.

What the human study found

Animal research explains a mechanism. It does not prove the mechanism plays out in people.

A 2022 study published in Nutrition Research measured plasma glutathione in 131 postmenopausal women and compared the women who had fatty liver, defined by a validated hepatic steatosis index, against the women who did not. The women with fatty liver had significantly lower glutathione, along with lower choline and betaine, two other nutrients tied to liver metabolism.

This is the strongest human data point connecting low glutathione to fatty liver specifically in women past menopause. One honest limit belongs right here. This was an observational, cross-sectional study. It shows that low glutathione and fatty liver travel together. It does not prove that low glutathione causes fatty liver.

Glycine availability plays a role too. Glycine is the amino acid that limits how much glutathione the liver can produce, and a 2019 review found glutathione synthesis running roughly 38 percent lower in people with metabolic dysfunction.

This is the part where the honest limits matter most, so we will say them plainly before anything else.

There is no human clinical trial testing shilajit in women with fatty liver. Nobody should tell you otherwise, and we are not going to pretend the research goes further than it does.

The human antioxidant trials

Shilajit has significantly raised glutathione in its own human clinical trials, run for other conditions. In the human bone density trial published in Phytomedicine in 2022, glutathione rose significantly in the women taking shilajit, alongside drops in markers of oxidative damage and inflammation. A separate 2016 human trial in people with type 2 diabetes found a similar antioxidant shift. Neither paper published an exact percentage worth repeating here, so the honest phrasing is that the increase was statistically significant, not that it doubled or tripled.

The animal liver study

In a 2020 study, researchers fed rats a high-fat diet for 12 weeks to induce a fatty liver model, then treated them with shilajit for 2 weeks. The treated rats showed significantly reduced liver enzymes, reduced triglycerides and cholesterol, reduced liver weight and fat buildup, and a restored antioxidant balance compared to untreated animals. A separate 2026 study found a shilajit extract protected mouse liver cells from a different kind of injury while regulating several of the same antioxidant markers, glutathione included.

That second study is a rat model, not a human liver disease trial. We are naming that limit clearly rather than letting a strong-sounding animal result imply more than it has earned.

Here is what we can honestly say, in order.

  • Estrogen appears to help protect the liver's glutathione defenses, based on animal research
  • Postmenopausal women with lower glutathione are more likely to have fatty liver, based on real human data in this exact population
  • Shilajit has raised glutathione in human trials run for other conditions
  • Shilajit has directly protected the liver in an animal fatty liver model
  • No study has yet put all four pieces together in one human fatty liver trial

That is a coherent mechanism worth knowing about. It is not the same thing as a completed human trial proving shilajit treats fatty liver, and this article will not dress it up as one.
