Where most calcium goes wrong

Getting calcium in is the easy part.
Getting it where it belongs is not.

Calcium does not steer itself. Two other nutrients decide whether what you swallow is absorbed at all, and where it ends up once it is. Most calcium supplements carry 1 of the 2. This one carries both.

Vitamin D is what allows calcium to cross out of your gut and into your bloodstream in the first place. Heaney's review of the interaction describes vitamin D status and calcium intake working on absorption together, not separately. Without enough D, a good share of what you swallow simply passes through.

K2 does the directing

Osteocalcin is the protein that binds calcium into bone, and it sits inert until vitamin K2 switches it on, a step called carboxylation. Matrix Gla protein is its counterpart out in your soft tissue. Both of them wait on K2. This is the step a plain calcium tablet leaves out entirely.

The dose size decides the share

Calcium absorption is not a straight line. The larger the single dose, the smaller the fraction your body actually takes up. 210 mg sits well inside the range where that fraction stays high, which is the same reason the standard advice is to keep any single calcium dose at or below 500 mg.

The absorption receipt

Why 210 mg and not 600.

A bigger number on the label is not a bigger number in your bloodstream. In 1990 Robert Heaney fed healthy women calcium loads from 15 mg up to 500 mg with a meal and measured what was actually absorbed. The share went down as the dose went up, every time.

At the smallest loads Heaney tested, absorption averaged 64.0% of the dose. At a 500 mg load it averaged 28.6%. Fractional absorption tracked inversely with the size of the load at P less than 0.001. That is the reason a 600 mg tablet does not deliver 3 times what a 210 mg capsule does, and it is why we did not chase a bigger number.

A 2017 analysis in JAMA pooled 33 randomized trials and more than 51,000 adults over 50 and found no significant reduction in hip, spine, or total fractures from calcium, from vitamin D, or from the 2 taken together.

What none of those trials tested was calcium and D with vitamin K2 alongside them. K2 is what activates the proteins that decide where calcium goes once it is in you, and that is the step the old stack has no mechanism for. We sell this as the companion piece to a bone routine, not as the routine itself, and we are not going to claim a fracture benefit we do not have evidence for.

Because the share your body absorbs falls as the single dose climbs. Heaney measured this directly in healthy women in 1990: the smallest loads averaged 64.0% absorbed and a 500 mg load averaged 28.6%. A bigger label number does not translate into proportionally more calcium reaching you, and it is the reason the standard advice has long been to keep any one calcium dose at or below 500 mg. You also still eat calcium every day. This is meant to sit on top of your diet, not replace it.

What is MK-7 and why does the form matter? +

MK-7 is the long-chain form of vitamin K2, and it is the form used in the human research because it stays in circulation far longer than the short-chain forms. The amount matters as much as the form. A dose-finding trial in postmenopausal women tested 0, 50, 100 and 200 mcg a day and found that 50 mcg was not enough, while 100 mcg was. That is where our 100 mcg comes from. To be precise about what that trial measured: it measured osteocalcin carboxylation in the blood, which is the mechanism, not bone density and not fractures.
