Why Inflammation Rises After Menopause and What It Does to Your Bones

When estrogen drops at menopause, the body's inflammatory signals rise on their own. A 2002 review in Endocrine Reviews found that loss of ovarian function brings spontaneous increases in IL-1, IL-6 and TNF-alpha, and that researchers have linked the rise to postmenopausal bone loss. Shilajit, a mineral-rich resin whose fulvic acid has been studied for its effect on these exact signals, was tested in a 48-week trial of 60 postmenopausal women with osteoporosis, where hip bone density rose on shilajit and fell on placebo.

What happens to inflammation when estrogen falls?

What happens to inflammation when estrogen falls?

Estrogen does more than manage a cycle. It also acts as a quiet brake on the immune system, holding down a small group of inflammatory messengers called cytokines.

Pfeilschifter, Koditz, Pfohl and Schatz reviewed the evidence in Endocrine Reviews in 2002. Their conclusion was direct. After the loss of ovarian function, IL-1, IL-6 and TNF-alpha rise spontaneously, and lab and clinical work ties that rise to postmenopausal bone loss.

When your estrogen dropped, the inflammation in your body went up. Researchers have tied that same inflammation to the bone loss.

Why this inflammation is easy to miss

It is not the red, swollen kind. It is a low, steady background signal, and it rarely announces itself.

Here are the places it tends to hide in everyday life.

  • Stiff joints in the morning that loosen up after an hour of moving.
  • Slow recovery after a workout that used to feel easy.
  • Poor sleep, which both feeds inflammation and is worsened by it.
  • A scan result that looks worse than the symptoms ever did.

How does inflammation turn into bone loss?

How does inflammation turn into bone loss?

Bone is not a fixed object. It is rebuilt all the time by two teams of cells, one that breaks old bone down and one that lays new bone in. Healthy bone depends on the two staying in step.

Inflammatory cytokines tip that balance. TNF-alpha and IL-6 push the bone-breaking team to work harder, partly by raising RANKL, the molecule that switches those cells on. Estrogen normally keeps the opposing brake, OPG, strong enough to catch RANKL first.

Three parts make up this loop.

  • Cytokines turn up the demand for bone breakdown.
  • RANKL is the signal that starts the breakdown.
  • OPG is the brake, and estrogen keeps it working.

When estrogen falls, the cytokines rise and the brake weakens at the same moment. That is why bone loss after menopause is faster than ordinary aging alone would predict.

Before menopause After menopause
Estrogen Present, holding the brake on Sharply lower
Inflammatory cytokines Held down IL-1, IL-6, TNF-alpha rise
RANKL against OPG Balanced Tilted toward breakdown
Bone turnover Rebuilding keeps pace Breakdown outruns rebuilding

What does fulvic acid do to inflammatory signals?

What does fulvic acid do to inflammatory signals?

Fulvic acid is the small organic acid that makes up a large share of purified shilajit resin. It also carries the minerals along with it.

Winkler and Ghosh reviewed the research on fulvic acid in 2018. They described it as modulating the immune system, lowering TNF-alpha and improving oxidative stress markers, with promise in chronic inflammation. That is a review of the field, not a bone trial, and it is worth reading that way.

Animal work on bone itself adds two data points.

  • Guler and colleagues (2025) used 28 rats with a healing shin-bone defect. Inflammation (TNF-alpha) fell as the shilajit dose rose, and the high dose gave the most new bone, 78.1% new bone area.
  • Alshubaily and Jambi (2022) gave shilajit to female rats with steroid-induced osteoporosis for 8 weeks. Blood calcium, phosphorus and calcitonin rose, and inflammation fell.
  • Azizpour and colleagues (2020) found shilajit lowered RANKL and raised OPG in human bone cells.

These are animal and cell results. They show the direction the mechanism points, and the human trial below shows what happened in women.

What did the human bone trial find?

What did the human bone trial find?

Pingali and Nutalapati published a randomized, double-blind, placebo-controlled trial in Phytomedicine in 2022, run at Nizam's Institute of Medical Sciences. It enrolled 60 postmenopausal women for 48 weeks. Their average hip T-score was -2.5, the osteoporosis line, and they were not allowed to take calcium or vitamin D.

Hip bone density rose on shilajit and fell in the women who did not take it. Every single woman who took it reversed her osteoporosis. OPG rose 57.3% and RANKL fell 9.8%, the same two signals the inflammation loop above acts on. The trial design and results are laid out on our clinical trial research page.

Shilajit is not a hormone. It supports the body's own estrogen signaling.

Zero serious adverse events appeared in the trial or in any human shilajit study. The wider library of bone studies sits on our shilajit bone research page.

What this changes about how bone loss is understood

Calcium alone cannot quiet a cytokine signal. A woman can take plenty of it and still have a skeleton under pressure from the inflammation described above.

Minerals are the raw material of bone, but the signals decide whether that material is used to rebuild or is pulled back out. That is why the trial above is worth noting. The women in it took no calcium and no vitamin D, and their hip density still rose.

The reading is cautious on purpose. One trial of 60 women is a strong start, not the last word, and the animal and cell work is direction, not proof in people.

Here is what the inflammation view adds to the picture.

  • Timing. Cytokines rise as estrogen falls, so the pressure starts early.
  • Reach. The same signals touch joints, sleep and recovery, not only bone.
  • Direction. A mechanism that calms the signal and supports estrogen signaling addresses the cause, not only the symptom.

The point of the inflammation research is simple. Bone loss after menopause is not only about calcium running low. It is a signaling problem, and signaling is where the research on fulvic acid and shilajit points.

Does menopause increase inflammation in the body?

Yes. A 2002 review in Endocrine Reviews by Pfeilschifter and colleagues reported that loss of ovarian function brings spontaneous rises in the inflammatory signals IL-1, IL-6 and TNF-alpha, and that laboratory and clinical work links the rise to postmenopausal bone loss.

How is inflammation connected to bone loss?

Inflammatory signals such as TNF-alpha and IL-6 push bone-breaking cells to work harder, partly through RANKL, the molecule that switches those cells on. When estrogen falls, that push has less opposition, and bone breaks down faster than it is rebuilt.

Does fulvic acid affect inflammation?

A 2018 review by Winkler and Ghosh described fulvic acid as modulating the immune system, lowering TNF-alpha and improving oxidative stress markers. Fulvic acid is the organic carrier that makes up much of purified shilajit resin.

Has shilajit been tested on bone inflammation directly?

In animal work, a 2025 rat study found inflammation (TNF-alpha) fell as the shilajit dose rose in a healing bone defect, and a 2022 rat study found lower inflammation alongside higher blood calcium in osteoporosis. These are animal studies, not human results.

What did the human shilajit bone trial find?

In a 48-week randomized, double-blind, placebo-controlled trial of 60 postmenopausal women with an average hip T-score of -2.5, every single woman who took shilajit reversed her osteoporosis, while hip density fell on placebo. No calcium or vitamin D was allowed.
