Background
Stress incontinence had long been attributed to childbirth injury and pelvic floor muscle weakness. This study tested a different hypothesis, that the connective tissue itself differs in women who leak.
Method
Periurethral tissue was taken from women with genuine stress incontinence and from continent controls, and collagen was measured directly. Critically, the incontinent women were premenopausal and none of them had ever given birth.
Conclusion
Women with genuine stress incontinence showed a significant deficit in total collagen, a decreased type I to type III ratio, and reduced mature cross-linking. The authors described it as a defect in their connective tissue. Because the cohort was young and nulliparous, the finding cannot be explained by childbirth, weight or exercise history.

Method
Postmenopausal women with urodynamically proven stress incontinence were randomised to oral oestradiol or placebo for 6 months. Periurethral tissue was biopsied before and after and analysed directly.
Conclusion
Total collagen fell and mature cross-links fell, while MMP-2, the enzyme that degrades collagen, rose. Two of the three deficits Keane measured in this tissue moved further in the wrong direction. This is a finding about a hormone, and it does not extend to non-hormonal interventions.

Keane measured three shortages in women who leak, and two of them are about type 1 collagen specifically, the thick load-bearing kind. A marker that is type-1-specific is therefore the right one to watch.
Method
Purified shilajit was given over 8 weeks and Pro-C1a1, a type-1-specific procollagen synthesis marker, was measured alongside hydroxyproline, a collagen breakdown marker.
Conclusion
Type 1 collagen synthesis rose substantially while the breakdown marker fell. Raising type 1 synthesis raises the numerator of the exact ratio Keane found decreased. Concordant with Das 2019 (PMID 31161927), which induced Col1A1 in a 14-week biopsy study in 45 women.
